The Craftsman team was a pleasure to work with, taking the time to understand our specific needs.  They were able to build a solution to meet the unique nuances of the data reporting requirements for the Diabetes Prevention Program. Rhode Island Parent Information Network

Improving Care Access and Outcomes w/ Centralized Reporting

The Rhode Island Parent Information Network (RIPIN) is a 501(c)(3) nonprofit organization that was established in 1991. RIPIN’s mission is to help individuals, parents, families, and children achieve their goals for health, education, and socio-economic well-being by providing information, training, education, support, and advocacy for person/family-centered care and system change.

The Rhode Island Department of Health’s mission is to prevent disease and protect and promote the health and safety of the people of Rhode Island. They partner with the Rhode Island Parent Information Network on many of their programs.

Purpose

The Rhode Island Department of Health (RIDOHseeks to build a central system for tracking data among health centers. These data were not historically captured in any one system. Salesforce was identified as the solution that could provide sufficient flexibility to meet their core functional needs.

The purpose of this engagement was to develop a Salesforce-based system that can help track registration and reportable data for programs as well as referrals between CDC recognized lifestyle change programs for type 2 diabetes prevention and the Community Health Network of RIDOH, hosted and staffed by RIPIN.

Project Details

RIPIN and RIDOH engaged Craftsman’s Nonprofit Practice to perform a Salesforce assessment. The outcome was a solution design that defined business requirements and needs by role and responsibility and outlined the steps to build the technical solution. As a result of the assessment, Craftsman recommended that RIPIN build out a portal that allowed their Community Health Network (CHN) partners access to Diabetes Prevention Program class and registration information in a secure way, and enabled them to enter attendance and key reporting information. Craftsman deployed and configured a new Salesforce Experience Cloud Community Site and it was deployed with their first partner.

The new Salesforce community is a place for RIPIN and RIDOH staff and partners to access class information for their Diabetes Prevention Program offering and was built with the flexibility to be expanded to other CHN programs. It offers one click reporting for CDC requirements and cross-organizational visibility into registration, referrals, and outcome data.

 Their data is now centralized in Salesforce, rather than in disparate systems and will streamline referral and communication processes, reporting to various entities including providers, referrers, internal stakeholders, and the CDC.  It enables collaboration and resource sharing between program providers while ensuring data security and integrity.

Ongoing Partnership

As the value of the community was demonstrated, RIPIN was interested in expanding the use of the platform to additional community health network programs and Craftsman was engaged to scope the expansion of the platform to include data for four additional CHN programs. A solution design was created and work has begun on the enhancement of the current system to accommodate of additional programs and program data.

Tools + Technology

RIPIN uses the Nonprofit Success Pack and a Salesforce Partner Experience. The Experience is a community site where program partners can easily login to view and enter their class participation data and export reports on it. FormAssembly and Constant Contact are used to post class events to their website, collect class requests, and registration for the classes.
