Community Health Centers
Community health centers increases access to critical services for all residents, regardless of background, status, or ability to pay. Central support resources provide training and technical assistance to tightly stretched providers. Local classes on topics like Diabetes Prevention, heart health, and wellness create access to information and resources in approachable spaces and language. Advocates work to ensure care is inclusive and equitable. These critical programs advocate daily for awareness, research, and support.
Measuring Impact + Outcomes
Craftsman works with Community Health organizations to collect and aggregate data to make a clear and specific case for support from local, state, and national representatives. We create compliant data collection forms to simplify information gathering, build processes that support impact measurement, and integrate tools to support training and technical assistance needs.
From the beginning of the project the Craftsman team listened to the concerns of the staff and built out an accurate assessment of our current organizational challenges and needs.
